Percent of incidents recorded 'after the fact'
Definition
On occasion incidents are raised 'after the fact', or after the occurrence of the event, and sometimes after the event (incident) is resolved. Sometimes it is a necessity to address an issue as it happens with a commitment to follow up with formal documentation.
